A state investigation has concluded multiple government agencies failed to protect or properly monitor a Fall River boy with intellectual disabilities before his 2020 death.

John Almond, 33, and his girlfriend, Jacyln Marie Coleman, 26, were both indicted on charges of second-degree murder and neglect of a disabled person resulting in serious bodily injury in the death of his 14-year-old son, David, in October.

The Office of the Child Advocate said there were multiple missed opportunities for prevention and intervention, and said critical errors were made by both the Department of Children and Families and Fall River Public Schools.

“Had anyone seen the boys in person, given the state of their starvation at the time that the EMTs arrived to take them to the hospital, that it would have been very evident that they were being starved to death,” said Massachusetts Child Advocate Maria Mossaides.

Ryanair staff in Spain made an autistic boy have a Covid test before boarding a flight, despite having an exemption letter, his mother has said.

Katy Hollingsworth, who was travelling to the UK from Valencia with her son Callum, 12, said he was “petrified”.

“They said if you don’t have a Covid test you can’t go home, so we had no choice,” said Ms Hollingsworth, who is from Harlow in Essex.

The airline has apologised and says it “regrets to hear of the stress” caused.

Ms Hollingsworth said his one previous test in January had been “a nightmare”, so his doctor had provided a medical exemption letter.

He did, however, have a test before he left the UK, but his family said they spent two months preparing him for this.

Government guidance says people with medical conditions which mean they cannot take a test, do not need one to travel to the UK, providing they “present a note from a medical practitioner at check in”.

The report also found that the DCF case managers were notified about the 14-year-old’s physical and behavioral deterioration, “but no action was taken by the DCF case management team to investigate the discrepancy between Davids presentation before and post-reunification.”

“There were virtual visits done with this family but in-person visits were rebuffed by the family and we did not catch it,” DCF Commissioner Linda Spears said. “We did not catch that there was purposeful efforts to avoid visits and avoid in-person contact.”

At least two people at DCF have been fired over this case, and the child advocate has recommended several steps including improved monitoring and better scrutiny of family reunification with more involvement from the juvenile court to see that nothing like this ever happens again. 

Almond and Coleman were arrested last Wednesday. They were arraigned Thursday in Fall River District Court on charges of caretaker neglect of a disabled person and drug possession. The caretaker neglect charges relate to the surviving brother, the district attorney said.

An arraignment date has yet to be set on the new charges.

When Baltimore and several other family members got to John Peter Smith Hospital, they were led to a private room. Baltimore knew something was wrong when hospital workers came in. Some were in tears, she said. Baltimore’s mother, Joyce Baltimore, asked the lead doctor to give her some good news.

“There’s no good news,” the doctor said.

Hernandez was pronounced dead at the hospital shortly after noon that day, according to records from the Tarrant County Medical Examiner’s Office. He was 21. Medical examiners haven’t yet determined his cause and manner of death.

Hernandez, who had autism, went to Boulevard Heights, a school in the Fort Worth district for disabled students. Teachers at Boulevard Heights restrained Hernandez earlier on the day he died, a district spokesman confirmed. An attorney with Disability Rights Texas, a Dallas-based disability rights group, said the organization is investigating reports that those restraints were improper and unnecessary.

Ebonie Baltimore said the district has refused to give her family any information about what happened in the last hours of her nephew’s life.

“I need to know what happened to him,” Baltimore said. “We deserve to know.”

Disability Rights Texas is also investigating the restraint of another Boulevard Heights student, fourth-grader Toni Crenshaw, in an incident that was caught on video in May. The video had been circulating on social media before someone called it to the attention of Toni’s mother, Sandra Crosby.

On Tuesday, police launched an extensive search for a 14-year-old boy with high functioning autism.

The boy, named William Wall, had gone missing during a jog near Launching Place, which is 54 km from the Melbourne metropolitan area. 

Now, Victoria Police have found his body near the Yarra Junction trail. Currently, the police are waiting for an autopsy report. However, Walls death is being treated as an accident since there is no evidence of foul play. 

According to the Walls parents, he went for a jog at around 6:45 AM. Apparently, he went there every morning for his daily exercise. 

Since he had high functioning autism, Wall was very rigid about punctuality. After he did not return, his parents called the police. Multiple officers and SES teams flooded the Launching Place locality. 

Walls older brothers and family friends tried to aid the search. However, officials did not allow them to help because of COVID-19 restrictions. Both brothers and friends expressed their frustration at this.

Walls friend, Flynn Cousens, stated, Id rather go outside the five kilometre radius and find him than stick to five kilometres and let him stay out there another night.

Emergency search teams were launched, searching the Warburton Trail at a 10-mile radius. Community volunteers were also aiding the search party.

Police found his body in a bushland, less than two kilometers away from his home.

PAULDING, MICH. A three-day search for a 17-year-old boy missing in Michigans Upper Peninsula ended with the discovery of the teens body, authorities said Monday.

The Ontonagon County Sheriffs Office didnt immediately reveal the circumstances of Cam Besonens death. Nor did they say where and who found the body. The search for Cam was centered near Paulding.

Cam, whose family said was autistic and non-verbal, was last seen Friday. He was wearing a long-sleeve shirt and jeans, clothes that dont offer much protection in cold weather. Jill Keeley-Besonen said her son had no sense of direction or how to yell for help.

Cams body was found about an hour ago, Keeley-Besonen posted on her Facebook page Monday evening. Thank you all who participated in the search. He is with Jesus!

During the past few days, over 400 volunteers along with local, state, tribal agencies searched for the teenager. Air operations including aircraft from the U.S. Coast Guard, Michigan State Police and Houghton Wing Civil Air Patrol also searched rugged, timbered terrain for the teen.

Our community mourns the loss of this young man, said Ontonagon County Sheriff Dale Rantala, adding rescuers had been committed to finding him and worked tirelessly during the effort.

Missing Autistic Boy Visiting Family Found Dead In Florida Pond

JACKSONVILLE A missing 4-year-old autistic boy who was visiting Jacksonville with his family from New York was found dead Monday night in a retention pond, police said.

Gavin Douyons family authorities they last saw the child around 5:30 p.m. on Monday. The Jacksonville Sheriffs Office used a helicopter, a dive team and a drone unit to search for the child. His body was located in the pond around 9:20 p.m., police said.

Theres no foul play. It appears to be a horrible accident, Assistant police Chief Brian Kee told news outlets.

The boy, who was nonverbal, had been playing outside with family members when he wandered off, Key said.

The family was visiting relatives for the Memorial Day weekend, authorities said.

Hes the second child with autism to die in a retention pond recently.

On April 25, five-year-old Mohamad Nour was found in a pond after his family reported him missing, news outlets reported.

Missing Autistic Boy Found Dead In Pond Near Jacksonville

JACKSONVILLE An autistic 5-year-old boy was found dead in a Florida pond shortly after he was reported missing over the weekend, officials said.

Investigators believe Mohamad Nour unlocked the front door and walked outside his home in Jacksonville early Sunday, news outlets reported.

Police were called at 8:48 a.m., and as many as 32 officers from the Jacksonville Sheriffs Office and 48 from the Jacksonville Fire and Rescue Department began looking for the boy, news outlets reported. They also used a drone, K9s and a helicopter in the search.

Crews in a boat found the boy in the pond just before 11 a.m. Sunday.

The body of missing teenager William Wall has been found in Victorian bushland with police stating his death was not suspicious.

The 14-year-olds body was found at Yarra Junction, east of Melbourne, near where he disappeared.

Victoria police can confirm the sad news that 14-year-old William Wall has been located deceased, a police spokesperson said in a statement on Wednesday evening. They said his death was not being treated as suspicious.

William disappeared before 7am on Tuesday morning, sparking an air and ground search involving about 100 personnel from Victoria police and the State Emergency Service.

He had left his Launching Place home to go for a walk in the Yarra Ranges. The teenager told his family he would be gone for 15 minutes but did not return.

William, who had high-functioning autism, left home without his mobile phone.

The air and ground search had involved police, including officers on horseback, as well as the SES and bush search and rescue.

Williams father, Shane Wall, was emotional earlier on Wednesday when he thanked those involved in the search effort. The police, the SES are doing a top job, and people are getting out there and his brothers mates are just all over the place, he said.

William has two older brothers, Jake and Harrison, and a younger sister, Sophie.

Shadow minister for police David Southwick said community volunteers were vital to the success of the search.

He said before William was found dead: While our police and emergency services perform an excellent job in cases such as these, residents with invaluable local knowledge must be allowed to assist.

Angry locals suggested William could have been saved.

One said on Twitter: “Poor autistic 14 year old William Wall died alone, disorientated, hungry & cold.

“Volunteers who knew the area were turned away. Dan Andrews mentioned not wanting people to congregate in crowds.

“Last time an autistic child was lost in the bush, a volunteer found him ALIVE!!”

In June, 14-year-old Will Callaghan – who has non-verbal autism – was found safe after two nights alone on Mount Disappointment.

More than 300 volunteers joined that search after Will became separated from his family on a hike north of Melbourne.

Maddox Ritch Who Disappeared From Park Died From Accidental Drowning

Maddox Ritch, the 6-year-old North Carolina boy with autism who disappeared from a park in September while out with his father, died from accidental drowning, according to officials.

Mecklenburg County Medical Examiner’s Office said in an autopsy report released Thursday, there was no evidence of trauma and the child’s body was “normally developed and nourished.”

Among them was neighbor, David Daniels, who helped find Jones a few times when he ran off before.

“Me and my girlfriend helped search yesterday for a few hours or whatnot. I was hoping that someone had just brought him home, hoping he was good. We didn’t think this was going to happen,” Daniels said.

Jones’ body was found in an apartment complex pool on Montfort Drive around 8 a.m. Sunday, and Dallas Fire-Rescue responded to recover the body, police said.

Dallas police did not release the name of the person who died, but the Dallas County Medical Examiner’s office identified Jones later Saturday. Athena Jefferson also said it was her nephew Jones’ body that was found in the pool.

An autistic teenager died after being restrained by Sheriff’s deputies following a violent episode between the teen and his dad, and now … his parents are suing the department, claiming they suffocated the kid to death.

Donna Lou and Daren Parsa have filed a lawsuit against the Jefferson Parish Sheriff in Louisiana over an incident last January that left 16-year-old Eric Parsa dead … which the parents allege happened because deputies on the scene sat on him for 9 minutes.

The lawsuit claims on January 19, 2020 … Eric and his family were leaving a laser tag facility at a shopping center, when for some reason he started attacking his dad in what his parents describe as a “meltdown” related to his condition.

Cops were called while Daren tried restraining Eric, and eventually, a few deputies arrived and handcuffed him while also shackling his feet … all the while holding him in a face-down position on the parking lot pavement as he continued to squirm and struggle.

Eric’s parents allege one deputy placed all his body weight onto the kid in the face-down position for nearly 10 minutes, and that at one point … another deputy reportedly put him into a chokehold, and never once moved him on his side so he could breathe properly.

The Jefferson Parish Sheriff’s Office denounced the lawsuit, saying it contained malicious and false claims about the deputies who tried to help that day … and that they’ll fight this in court.

Coroner calls for changes in how schools treat cases of absent children following tragic death of Chadrack Mulo

A four-year-old boy, who was unable to call for help or feed himself, starved to death two weeks after his mother collapsed and died suddenly in the flat they shared, a coroner has said. Chadrack Mulo was found clinging to his mothers body about two days after his own death.

The childs teachers became concerned when he did not turn up for school. They tried to call and visited the familys home, in Hackney in east London, twice but could not get in, the inquest into his death found.

The likelihood is that Chadrack lived alone in the family home for over a fortnight after his mothers death. He was found a couple of days after his own death, with his arms around her body. She was by then very decomposed, the coroner, Mary Hassell, said.

She said that Chadracks mother, Esther Eketi-Mulo, died suddenly after an epileptic fit at some point in the first few days of October last year. Chadrack, who the coroner said had learning difficulties and had autism spectrum disorder, was alone in the flat with his mothers body thereafter and was unable to look after himself. He died a fortnight later of dehydration and starvation. He was then found within approximately 48 hours, Hassell, based at St Pancras coroners court in north London, said.

She added that Chadracks east London school now collects phone numbers for three family members, rather than just one.

Last August, a 23-year-old young man, Elijah McClain, was walking home from a convenience store close to his home in Aurora, Colorado. He had just purchased some iced tea for his brother. He was wearing a ski mask as he often did to keep his face warm because of an underlying blood condition.

A resident called the police because they thought he was acting suspiciously. He had not committed any crime and was doing nothing illegal. The police arrived and confronted him. A struggle ensued during which he was placed in a chokehold.

When paramedics arrived at the scene, they injected him with twice the usual dose of ketamine for his weight. He subsequently went into cardiac arrest on the way to the hospital and was declared brain dead a few days later.

During the altercation with the police, Elijah can be heard sobbing, apologizing for vomiting after the police choked him, telling them that he doesnt like being touched and is an introvert, and telling them he loves them. By all accounts, Elijah was a lovely young man with a kind and gentle spirit. One of his massage therapy clients said he had a child-like spirit who lived in his own little world. He just was who he was.

Recently, a close family friend described to my wife how deeply affected she had been by Mr. McClains death. Her son, who is about Mr. McClains age, has autism. As she listened to the voice recordings of the encounter, she was moved to tears.

Body Of Missing North Carolina Boy Maddox Ritch Believed To Be Found

Parents of ‘severely autistic’ teen killed in JPSO custody file civil rights lawsuit

Maddox, who was also nonverbal, vanished from Rankin Lake Park in Gastonia on Sept. 22. He was walking with his father, Ian Ritch, and another adult, police previously said. His body was discovered partially submerged in a creek located more than a mile east of the park by a searcher five days after he disappeared.

Ian Ritch said at the time that he was walking with his son when Maddox took off running behind a jogger. Ritch said he tried to catch up to Maddox but didn’t know in which direction he ran.

According to the medical examiners report, Maddox was found wearing the same clothes he disappeared in. It also stated that the child’s body had multiple wounds on his face, upper neck, lower abdominal, and pelvic region and on his right upper arm most likely from “postmortem animal predation.”

An X-ray also showed no signs of bone injuries, and blood samples returned normal results.

“In conjunction with investigative information at this time, which gives no indication of other than an accidental drowning, it seems reasonable to conclude that the likely cause of death is drowning,” the report read.

The Gastonia Police Department, which helped the FBI in searching for Maddox, released a statement Thursday saying they don’t expect to file criminal charges in connection to Maddox’s death.

AN autistic boy lost in the Australian bush has been found dead – after cops turned away volunteers because of a coronavirus curfew.

More than 50 locals gathered hoping to help rescuers scour the countryside east of Melbourne for missing William Wall, 14.

But police told them to go home, asking them to spread the word on social media instead of searching the walking trails.

Today more than 30 hours after he went missing, William was found dead at Yarra Junction.

Victoria police said: His death is not being treated as suspicious and police will prepare a report for the coroner.

Police would like to thank the media and all those involved in the search.

William, a keen runner with high functioning autism, left his home in Launching Place at 6.45am on Tuesday.

He told his family he was going for a jog and would be back in 15 minutes.

Dad Shane Wall said he may have become disoriented without food or his phone.

He said earlier on Wednesday: Its absolutely the first time hes got lost.

Hes a kid that sticks to himself, does all his school work, so this is really out of the ordinary.

Police officials said: ‘We have determined that Thomas was never in the driveway that morning, and he suffered head and facial injuries that were not consistent with his father’s account’

LONG ISLAND, NEW YORK: A New York Police Officer and his fiancée were arrested for murder on Friday, January 24, over the death of the officer’s eight-year-old autistic son.

The 40-year-old officer Michael Valva, and his 42-year-old fiancée Angela Pollina were charged with second-degree murder after leaving Valva’s son, Thomas, in their garage at their Long Island home, according to a CNN report.

After finding the boy unresponsive, Valva called the police saying the kid had fallen in the driveway. The site reported that he was performing CPR on Thomas when the police arrived. He was taken to Long Island Community Hospital where doctors pronounced him dead.

Talking about the incident, Suffolk County Police Commissioner Geraldine Hart, in a press conference, said: “We have determined that Thomas was never in the driveway that morning, and he suffered head and facial injuries that were not consistent with his father’s account.”

He added that the temperature that night was 19 degrees and Thomas’s body temperature was 76 degrees. The medical examiner called his death a homicide and ruled hypothermia as a factor. 

According to Detective Sophia Mason, Valva, a police officer in New York since 2005, has been suspended without pay. The two have pleaded guilty but are still in custody.

A man now faces a homicide case because a boy he allegedly beat has died in the hospital, according to a WCCO report. The young victim reportedly lived with autism.

Reggie Bethel, 56, faces charges of second-degree unintentional murder, first-degree manslaughter, and second-degree manslaughter for killing an 11-year-old boy, authorities said. The suspect lived with the child and the boys mother.

Cops in Fergus Falls, Minnesota responded to a call on July 29 regarding the boy sustaining a head injury. Bethel and the childs mother both allegedly lied to police at first. He told investigators that the boy slipped while cleaning up dog urine and feces, and she had a similar story, authorities said.

Investigators did not buy it, at least after doctors determined the injuries were too severe to be an accident. The 11-year-old needed surgery for a traumatic brain injury. He remained intubated for nine days before dying on Aug. 7, police said.

At one point, the childs mother changed her story to claim Bethel told her to lie, cops said. In truth, Bethel hit the childs head against the wall and floor, she said. This happened often and was a form of discipline.

He allegedly described himself as having an anger problem, said he knew what he did was wrong, and said that the boy never attacked him.

A neighbor, who was not named, told WDAY that Bethel would throw tools around the yard out of frustration.
