Helping The Person To Understand The Changes
Supporting autistic people and people with a learning disability requires skill and time in what can sometimes be complex situations. The pandemic has increased confusion, fear and anxiety for everyone, and inevitably it will continue to bring additional challenges. This may include the requirement to self-isolate if the person develops symptoms or has not yet been fully vaccinated and has been in close contact with someone who tests positive. In such situations there may be a need for detailed planning where people are used to specific routines, and are unsettled by change.
Each person is an individual. It is important therefore to think about what the person you are supporting enjoys normally how they choose to spend their time, who is important to them, what strategies and plans typically work when things are not going well and to continue to provide calm and reassuring support.
Much has been changed by the coronavirus, but the personality, preferences and interests of the person you are supporting will likely not have done. So, make sure the person is included in discussions and decisions, so theyll have the best chance of understanding what is happening, and of staying in control of their lives as much as possible.
If the person you support does need to change their routine and stay at home for a time, it may help to think about what strategies have been used before if the person was going, for example, on holiday or to hospital.

Potential Shortcomings And Limitations Of The Interpretations
The EFAAR study is the first with a multicentric and prospective design carried out on adult patients with ASD-ID in France. With only 63 patients, our ASD-ID cohort may not be representative of all people with ASD-ID in France. In addition, our patients were recruited from medico-social institutions. They were not hospitalized at the time of the assessment and were considered to be stable. Nevertheless, we may have selected individuals with more severe ASD-ID, as shown by the mean DQ. Thus, the high comorbidity burden and rates found in our study should be interpreted with caution, because it refers to a very specific population with a very severe ASD-ID disorder. The severe ID observed in our population could be the most important cause of the observed high comorbidity burden.
The homogeneity of the profound ID prevented us from using the level of ID as a variable in univariate and multivariate analyses. Thus, the results of this preliminary study need to be confirmed in a larger cohort of ageing people with ASD, with or without ID, to better understand the effect of ID on the comorbidity burden.
The colinearity of the clinical characteristics and certain comorbidities also make interpretation of the univariate analyses difficult.
Furthermore, there are no previous studies concerning ageing with ASD-ID. Thus, we can compare our results only with those obtained for ageing people with ID.

What Is Severe Autism
Unfortunately, there is no standard definition for the diagnosis of autism spectrum. The research Ive done involves a couple of key points to categorize someone as on the severe end of the spectrum. Their communication and social skills are usually very impaired, so either not speaking at all, or minimally verbal. Not conversational and not able to fully express their wants and needs. Their IQ score, if thats been done, is usually lower than 70, which indicates what we used to call mental retardation, and now we call intellectual disability.
Kids with severe autism have an inability to be independent with safety, staying alone in the house or out in the community, and taking care of all their physical needs like toileting, showering, and dressing. They would need some kind of supervision or assistance in those areas usually because of their ability or inability to communicate fully.
A lot of times kids with severe autism also have problem behaviors and sometimes major problem behaviors including repetitive behaviors, aggression, self-injurious behavior, and property destruction. All of these things rolled into one presents really big challenges, especially as the child grows older.

Assessment Of Baseline Comorbidities
Screening for 49 diseases was performed.
The Dementia Screening Questionnaire for Individuals with Intellectual Disabilities was completed by the referent health worker of the participant to screen for neurocognitive conditions. The first part of this questionnaire targets the participants abilities, the second targets the behavior and symptoms usually associated with dementia in people with ID, and the third includes 10 comparative questions. Dementia is suspected for a total score of 20.
Other comorbidities were evaluated by examining the medical record , the last biological checkup , and medical examinations of the participant carried out by one of the authors .
Among the 49 diseases, 44 were grouped into 14 categories of chronic health conditions to provide an overview of the prevalence of the comorbidity categories.
