Imaging studies in patients with autistic disorder who exhibit head banging may show enlargement of the diploic space in the parietal and occipital bones, with loss of gray matter adjacent to the bony changes. These findings resemble those of posttraumatic encephalopathy in athletes in contact sports and professional boxers .

    A major goal of this research and other autism imaging studies is to find ways to use brain imaging to distinguish between autism and other developmental disorders with similar early symptoms, such as speech delay and attention deficit hyperactivity disorder.

    Behavioral pediatrician Andrew Adesman, MD, tells WebMD that it is not yet clear if imaging will prove useful in the clinical setting.

    Adesman says the fact that the Stanford study did not include children younger than 8 or children with Aspergerâs and other non-autism disorders limits the interpretation of the findings.

    Autism imaging researcher Nicholas Lange, ScD, says it remains to be seen if brain imaging can help distinguish between autism and other developmental disorders since most studies have compared autistic children to those who were developmentally normal.

    Lange is an associate professor of psychiatry and biostatistics at Harvard Medical School.

    Although he says he is optimistic that brain imaging will one day prove clinically useful, Lange adds that much more research is needed.

    What if I told you that we can now identify babies who are going to develop autism based on a simple brain scan? This, in essence, is the seductive pitch for a study published last week in the journal Nature, and making headlines around the world.

    Early identification and diagnosis is one of the major goals of autism research. By definition, people with autism have difficulties with social interaction and communication. But these skills take many years to develop, even in typically developing children. Potential early signs of autism are extremely difficult to pick out amidst the natural variation in behaviour and temperament that exists between all babies.

    A brain scan for autism would be a major step forward. But is the hype justified? Are we really on the brink of a new era in autism diagnostics? Without wishing to detract from the efforts of everyone involved in the study, its important to look at the results critically, both in terms of the scientific findings and their potential implications for clinical practice.

    So Hazlett and colleagues tried a different approach, calculating the volume and surface area for 78 different regions within each infants brain. They did this twice: once for the 6 month scan and again for the 12 month scan, giving them 312 datapoints, or features, for each baby.

    A computer algorithm called a classifier can distinguish between adults with and without autism by studying brain scans.

    The software found 16 key connections that allowed it to tell, with high accuracy, who had been traditionally diagnosed with autism and who had not. The team developed the classifier with 181 adult volunteers at three sites in Japan and then applied it in a group of 88 American adults at seven sites. All the study volunteers with autism diagnoses had no intellectual disability.

    Individuals with autism spectrum disorder show deficits in social interactions, communication and behavior. These deficits are accompanied by widespread differences in brain structures and activation patterns between individuals with autism and healthy individuals. Magnetic resonance imaging of healthy volunteers and individuals with autism show differences in the volumes of multiple brain regions including:

    • The frontal cortex, which is involved in social and cognitive functions tends to be thicker.

Changes in brain activity levels while performing the test are measured by evaluating changes in blood flow to different brain regions.

A separate imaging technique was then used to reconstruct these scans into 3D images that could be assessed for structure, shape and thickness — all intricate measurements that reveal Autism Spectrum Disorder at its root. By studying the complex and subtle make-up of grey matter in the brain, the scientists can use biological markers, rather than personality traits, to assess whether or not a person has ASD.

    The research was undertaken using the A.I.M.S. Consortium, which is funded by the MRC. Support funding was also provided by the Wellcome Trust and National Institute for Health Research .

    What The North Carolina Study Discovered

    Eye scans that can identify child autism risk

    Scientists at the University of North Carolina did brain scans using an MRI to detect signs of autism in babies. They focused their study on infants with older siblings on the spectrum. Parents who have at least one child with autism have a two to 18 percent risk of having a second.

    The North Carolina researchers started the imaging scans at 6 months of age. They conducted a second scan at age one and a third at age two in order to monitor the developing brain.

    The MRI scans showed all the infants had significant increases in brain volume during that first year, yet, many of these babies started to show signs of autism later on. They were able to take the multiple views and make comparisons that allowed them to predict which babies would become autistic.

    This is not the first time the MRI has been linked with diagnosing autism. In 2011, scientists at Stanford University used brain imaging to study the network that controls social communication and self-regulation and found it different in children with autism.

    Using the MRI imaging, they could easily pick out the brains of children already diagnosed with autism from those of children with more typical development patterns. This method had a 92 percent accuracy rate. The North Carolina study, though, is the first to use the MRI as a diagnostic tool to predict autism prior to the development symptoms.

    What Were The Basic Results

    Using this method, the study was able to identify individuals with ASD with a sensitivity of up to 90% .

    However, the accuracy of the results varied according to the measurements used. The computer diagnoses were more accurate using measurements from the left hemisphere of the brain, with individuals with ASD being correctly identified in 85% of all cases, when all five measures were taken into account. The highest accuracy of 90% was obtained using a measurement of cortical thickness in the left hemisphere.

    On the right hemisphere, the assessments were;not as;accurate, with individuals with ASD being correctly classified in 65% of all the;cases.

    Specificity was also very high. Of the control group, 80% were correctly classified as controls.

    In the ADHD control group, information from the left hemisphere was used to correctly identify 15 of the 19 individuals with ADHD , while four of these individuals were incorrectly allocated to the ASD group. Classifications using the right hemisphere were less accurate.

    TUESDAY, July 17, 2018 — Children with autism show abnormalities in a deep brain circuit that typically makes socializing enjoyable, a new study finds.

    Using MRI brain scans, researchers found that kids with autism showed differences in the structure and function of a brain circuit called the mesolimbic reward pathway.

    That circuit, located deep within the brain, helps you take pleasure in social interaction — something that people with autism struggle with, the study authors explained.

    Experts said the findings, published July 17 in the journal Brain, offer insight into what’s happening in the autism-affected brain.

    One of the hallmarks of the disorder is difficulty with recognizing and responding to other people’s social cues. The new study suggests that, due to brain wiring, those interactions just do not feel as rewarding to people with autism.

    If a young child does not feel the inherent pleasure of socializing, the researchers said, he might avoid it — and then miss the chance to develop complex social skills.

    However, the findings do not definitively prove that the brain abnormality causes social difficulties, said Kaustubh Supekar, a research scientist at Stanford University School of Medicine who worked on the study.

    The researchers scanned children who were ages 7 to 13. And it’s possible, Supekar said, that the brain circuit did not develop normally because the children lacked years of typical social interactions.

    Brain Growth In Infants With Autism

    Significant brain growth happens when the child is between 6 months and 12 months of age. However, one study has shown that brain growth is even more accentuated in infants with autism, compared to those without autism, and these changes can be detected as early as 6 months of age through MRI scans. The rapid brain growth in infants with autism accounts for the larger brain people diagnosed with autism usually have.

    During the study, MRI scans of the participating infants were taken when they were 6, 12, and 24 months of age and compared for differences. Their study revealed that between 6 and 12 months of age, the growth rate of the brain surface in children with autism is faster compared to those without autism. It also revealed that, between 12 and 24 months of age, the overall brain size of children with autism grows faster and is bigger compared to those without autism.

    Although its still unclear exactly how the difference in brain size affects infants, but the rapid production of brain cells and the cascade of brain changes may have contributed to the development of autism by the time the child turns two years old.

    For children with autism spectrum disorder , early diagnosis is critical to allow for possible interventions at a time when the brain is most amenable to change. But thats been tough to implement for a simple reason: the symptoms of ASD, such as communication difficulties, social deficits, and repetitive behaviors, often do not show up until a child turns 2 or even 3 years old.

    Now, an NIH-funded research team has news that may pave the way for earlier detection of ASD. The key is to shift the diagnostic focus from how kids act to how their brains grow. In their brain imaging study, the researchers found that, compared to other children, youngsters with ASD showed unusually rapid brain growth from infancy to age 2. In fact, the growth differences were already evident by their first birthdays, well before autistic behaviors typically emerge.

    Autism spectrum disorder includes a range of developmental conditions, such as autism and Asperger syndrome, that are characterized by challenges in social skills and communication. Scientists have long known that teens and adults with ASD have unusually large brain volumes. Researchers, including Heather Hazlett and Joseph Piven of the University of North Carolina, Chapel Hill, found more than a decade ago that those differences in brain size emerge by about age 2 . However, no one had ever visually tracked those developmental differences.

    It may be possible to detect autism in babies before their first birthdays, a much earlier diagnosis than ever before, a small new study finds.

    Using magnetic-resonance imaging scans, researchers at the University of North Carolina were able to predict with an 80 percent accuracy rate which babies who had an older sibling with autism would be later diagnosed with the disorder.

    The brain imaging scans, taken at 6 months, at 12 months and again at 2 years, showed significant growth in brain volume during the first year in babies who would later meet the criteria for autism, such as not making eye contact, delaying speech or other displaying other developmental delays.

    “It’s the first marker of any sort, brain or behavior, in infants, to predict which individuals would be classified as autistic at 24 months of age,” said Dr. Joseph Piven, senior author of the study and director of the Carolina Institute for Developmental Disabilities in Carrboro, North Carolina. The report was published Wednesday in the journal Nature.

    Parents who have a child with autism have a 2 percent to 18 percent increased risk of having a second child who is also affected, according to the Centers for Disease Control and Prevention.

    The MRI scans were taken on 109 high-risk babies who had older siblings with autism and 42 infants with no family history of autism, while they slept at four centers across the country.
