The first-ever device to treat pediatric attention-deficit hyperactivity disorder received clearance from the U.S. Food and Drug Administration in April. The external Trigeminal Nerve Stimulation system offers an alternative treatment option for more than 6 million American children with an ADHD diagnosis.
The current standard of treatment for the disorder involves a combination of medicationtypically a stimulant such as an amphetamineand behavioral therapy, depending on a childs age, say clinical practice guidelines from the American Academy of Pediatrics .
The eTNS system, designated for use in children ages 7 to 12 under the supervision of a caregiver, is a small electronic device that delivers low-level stimulation to the brains trigeminal nerve. Each night, electrodes applied to the forehead deliver eight hours of treatment while a child sleeps. Already studied as a potential aid for depression, epilepsy and post-traumatic stress disorder in adults, the eTNS system has now been shown to produce a statistically significant reduction in ADHD symptoms in a clinical trial 30045-0/fulltext” rel=”nofollow”> Journal of the American Academy of Child & Adolescent Psychiatry, Vol. 58, No. 4, 2019).

How Does Etns Compare To Other Treatments For Adhd
While eTNS hasnt been compared head-to-head with other ADHD treatments, Loo notes that the improvements are on par with those of the widely-used, non-stimulant ADHD medications atomoxetine and guanfacine.
But stimulant ADHD drugs are generally regarded as more effective. The eTNS effects are bigger than those found in other studies for non-drug therapies like neurofeedback, child and parent training, omega-3 fatty acids, and cognitive therapy.
Side effects were mild, including an increase in appetite and weight, higher pulse, fatigue, and headache. But none of the participants in the 2019 study had serious reactions or stopped using the device. Loo and a team at UCLA continued to study eTNS for ADHD, finding that kids who responded best tended to have lower scores in spelling and math and more problems with working memory than non-responders, in the Journal of the American Academy of Child & Adolescent Psychiatry.
While more research is needed, she notes that it may be one of the better non-medication treatment options for ADHD for kids who dont respond to medication or have serious side effects.

The American Academy of Pediatrics notes that clinicians should titrate doses of ADHD medication to achieve maximum benefit with tolerable side effects.6
If adverse reactions persist, the clinician can make the switch to another stimulant, or to a nonstimulant.

Device And Digital Therapeutics
The FDA has recently approved a device and a digital treatment for ADHD in children. Clinicians should note that the standard for FDA approval is lower for devices than for drugs. Drugs must prove both safety and efficacy, while devices need only prove safety.
The first device is a trigeminal nerve stimulator. Unlike the vagal nerve stimulator used to control epileptic seizures, this therapy does not require surgery. The trigeminal nerve stimulator sits on the face. A small-scale study using data from the device manufacturer suggests that about half of pediatric ADHD patients responded to use of this device.
The FDA has also approved use of a video game called EndeavorRx to treat ADHD. The sensory stimulus and motor challenges of the game target neural pathways that govern focus and attention. A study sponsored by the developer suggests that the prescribed use of the game does improve attention and has few side effects.
Either or both devices may prove to be valuable adjuncts to medication and behavior management. More evidence of effectiveness would be helpful in guiding clinical decisions.
Behavior Therapy Including Training For Parents
ADHD affects not only a childs ability to pay attention or sit still at school, it also affects relationships with family and other children. Children with ADHD often show behaviors that can be very disruptive to others. Behavior therapy is a treatment option that can help reduce these behaviors it is often helpful to start behavior therapy as soon as a diagnosis is made.

Fda Approves New Nonstimulant Option For Adults With Adhd
Adults living with attention deficit hyperactivity disorder saw new hope this weekend as the U.S. Food and Drug Administration expanded the indication of Supernus‘s Qelbree to patients ages 18 and older.
Qelbree is a novel nonstimulant that is taken once a day for full exposure. Results from the Phase III trial in adults demonstrated efficacy, tolerability and safety, with no evidence of abuse potential. Notably, Qelbree is the first FDA approval of a novel non-stimulant treatment for ADHD in 20 years.
The randomized, double-blind, placebo-controlled study gave patients a daily flexible dose of 200 mg to 600 mg and measured outcomes based on the Adult ADHD Investigator Symptom Rating Scale .
At the end of the trial, Qelbree showed the ability to meet the primary endpoint of reduction in the change from baseline based on AISRS scores and a statistically significant improvement in inattention and hyperactivity/impulsivity symptoms. After six weeks, the study also met the key secondary endpoint of a significant change from baseline based on the Clinical Global Impression – Severity of Illness scale.
The drug was first approved for the 6- to 17-year-old age range in April 2021.
Around 16 million children, adolescents and adults are diagnosed with ADHD in the U.S. alone, and up to 90% of those who have the condition as children carry it into adulthood.

What To Ask Your Childs Doctor About New Adhd Medications
The US Food and Drug Administration approved two new medications for ADHD in the spring of 2021: Azstarys, a stimulant medication, and Qelbree, a nonstimulant. Both can be prescribed for children age six and older.
Newly approved medications to treat ADHD can be promising, and these medications may fill in the gaps where other medications are less effective or have intolerable side effects. Parents should learn as much as they can about any new medication when it is prescribed, however.

Its best to ask some specific questions about newly approved ADHD medications, including their cost, says Max Wiznitzer, MD, a pediatric neurologist in the Neurologic Institute at University Hospitals Rainbow Babies and Childrens Hospital in Cleveland. Many medications used to treat ADHD are available in generic form, which are more cost-effective and more likely to be covered by insurance. Dr. Wiznitzer recommends that parents ask for generic medications first instead of newly approved ADHD medications, because these new medications tend to be more costly third-tier medications and most often not covered by Medicare.

U.S. regulators have approved the first new drug in over a decade for children with ADHD, which causes inattention, hyperactivity and impulsivity.
The Food and Drug Administration late Friday OKd Qelbree for treating attention deficit hyperactivity disorder in children ages 6 to 17. It comes as a capsule thats taken daily.
Unlike nearly all other ADHD medicines, Qelbree is not a stimulant or a controlled substance, making it harder to abuse than older drugs. Thats been a problem with earlier ADHD treatments like Ritalin, nearly all of which contain the stimulants amphetamine or methylphenidate.
Qelbree, developed by Supernus Pharmaceuticals of Rockville, Maryland, carries a warning of potential for suicidal thoughts and behavior, which occurred in fewer than 1% of volunteers in studies of the drug.
Supernus wouldnt disclose the drugs list price, but its sure to be higher than the many cheap generic ADHD pills.
ADHD affects about 6 million American children and adolescents. For many, problems include trouble paying attention and completing tasks, fidgeting and impulsiveness.
Experts say the drug may appeal to parents who dont want to give their child stimulants.

Promising New Treatment For Adhd
Andrew Cutler, MD, shares insights on the results of a Phase III trial that shows great promise in the treatment of ADHD.
According to the Centers for Disease Control, about 6.1 million children in the United States have been diagnosed with attention-deficit/hyperactivity disorder. While the number of available agents has grown over the years, many options have harsh side effect profiles or do not fully address the symptoms.
Results from a phase III randomized, placebo-controlled study of a new agent showed promise of a safe and effective new treatment option. The results appear in Clinical Therapeutics.
In this Psychiatric Times Mental Health Minute, Andrew Cutler, MD, shares highlights of the SPN-812 study. Cutler is a trial investigator and clinical professor of psychiatry, SUNY Upstate Medical University.
Cutler explains the agent, a multimodal serotonergic and noradrenergic modulating agent and non-stimulant medication, was successful in in improving symptoms of ADHD in children and met both primary and secondary outcome measures. In addition, he notes efficacy was seen as early as one week after initiating treatment, which is unlike many of the other treatments available. The agent also appeared to be well tolerated.
According to a press release, SPN-812 has PDUFA target action date of Nov. 8, 2020.

At the 43rd National Conference on Pediatric Health Care for nurse practitioners in Dallas, Texas, a look at the newest medications for children with attention-deficit/hyperactivity disorder.
Erin OConnor Prange, MSN, CRNP, from Childrens Hospital Philadelphia in Pennsylvania, began her session by explaining, Drugs for ADHD have been in the market for more than 45 years, beginning with Ritalin, which was approved in 1955. For the purposes of this discussion, however, we are going to focus on medications that have been approved in the past 5 years.
According to a national survey in 2016, roughly 6 million children in the United States have been diagnosed with ADHD. Boys are diagnosed twice as frequently as girls. Also in this survey, it was reported that 77% of these children were receiving some form of treatment, and roughly 32% of that group received both behavioral and medication treatment.
When discussing what ADHD looks like, Prange explained that the main component is poor executive functioning in tasks that include planning, time management, attention, organization, self-control, and others. Additionally, during the months that children were learning remotely due to the COVID-19 pandemic, impaired communication, eye strain, distractions, and anxiety were also reported, largely as a result of the struggle with virtual and hybrid learning.

How Is Adhd Medication Dosed

Even with the FDAs guidelines, the average consumer may find labels on ADHD medications confusing. Take the ADHD medication Concerta. Designed to last 12 hours, Concerta has a sponge on the bottom of the capsule, medication on top, and a tiny hole above the medication. As the capsule passes through the gastrointestinal tract and absorbs moisture, the sponge expands and pushes the medication out of the hole.
The number value assigned to each dose is where the confusion tends to lie. Take Concerta 18 mg. If the goal is to release 5 mg consistently every four hours over a 12-hour period, then there needs to be 15 mg in the capsule. However, it takes time for the sponge to become moist enough to start to expand. So an initial release of medication is needed until the sponge starts working. Researchers figured out that it should be 3 mg. Thus, to release 5 mg over 12 hours, one needs the initial 3 mg, plus 5 mg every four hours during the 12 hours. The total amount of medication is 18 mg. Thats why the medication is called Concerta 18.
