Why It Is Important To Do This Review

My 2-YEAR JOURNEY With Wellbutrin/Buproprion For ADHD & Depression: The Pros & Cons

The high ADHD prevalence rate, incomplete response and/or adverse drug reactions to stimulants, as well as the risk of stimulant abuse mean that pharmacotherapeutic alternatives to stimulants, such as bupropion, should be explored.

Although buproprion is widely used as a treatment for ADHD, data from randomised controlled trials regarding its effectiveness are lacking . It is of both clinical and research interest to document the efficacy of buproprion for ADHD, as it might generate new avenues of research that inform us of the aetiology of ADHD . It is likely that there is a differential response to stimulant and nonstimulant drugs in adults with ADHD. Patients may prefer one treatment to the other . A major challenge for the future is to develop strategies to identify a priori those people who will prefer a stimulant over a nonstimulant drug. At present, there are a lack of empirical data to guide such treatment selection.

A nonscheduled agent such as bupropion does not produce euphoria and so may be used in patients who have a history of illicit drug use and abuse . Differing patterns of comorbidity and symptom heterogeneity pose different treatment challenges. As psychostimulants are relatively contraindicated in people with ADHD and comorbid conditions, such as depression, anxiety and substance misuse, it is vital to explore nonstimulant treatment options.

Bupropion For Attention Deficit Hyperactivity Disorder In Adults

Review question

We reviewed the evidence on the effects of the drug bupropion in adults with ADHD. We also reviewed adverse effects .

Background

ADHD is a brain disorder marked by an ongoing pattern of inattention, impulsivity and/or excessive activity, which interferes with functioning or development. Some people with ADHD only have problems with one of the behaviors, while others have a combination. Bupropion is a drug that is registered for the treatment of depression and for stopping smoking, but is also used to treat ADHD. People with ADHD are usually prescribed stimulant drugs such as methylphenidate and amphetamines. However, not everyone responds well to stimulants. Some people cannot tolerate stimulants because of side effects. Others have medical conditions such as psychiatric conditions or tic disorders that make it not possible to use stimulants. Other people may not want to use stimulants, as these are controlled substances. Nonstimulants such as bupropion are therefore sometimes used instead. Its effectiveness in treating ADHD is not yet known.

Effectiveness And Safety Of Bupropion

To assess bupropion effectiveness, the Clinical Global Impressions-Depression-Severity and Clinical Global Impressions-Depression-Improvement scores were determined by two child psychiatrists. In subjects with comorbid ADHD, the CGI scores for ADHD were separately rated. One psychiatrist who was involved with the treatment and another who was not independently rated the CGI. Discrepancies between the reviewers were discussed and resolved by consensus. The CGI-Depression-S and CGI-ADHD-S scores were obtained at baseline and the CGI-Depression-S, CGI-ADHD-S, CGI-Depression-I, and CGI-ADHD-I scores at the 4th, 8th, and 12th weeks . Treatment response was defined as a CGI- Depression-I score of 2 by the 12th week of treatment. Safety was assessed at each visit based on spontaneous reports from the patients or their parents. AEMS was determined based on Goldberg and colleagues criteria .

  • Non-stimulant medication: The FDA has approved non-stimulant medication for ADHD, including atomoxetine , guanfacine , and clonidine . These may provide fewer side effects than stimulants, or be safe for people with health conditions that are contraindicated for stimulant medication.

Wellbutrin Vs Adderall: Whats The Difference

ADHD affects nearly 4% of adults and 8% of children in the United States.

Adderall is one of the medications of choice for managing the symptoms of ADHD.

However, its not the only medication that is prescribed.

In some cases, patients may not tolerate the stimulant drug and are recommended another medication, like Wellbutrin.

While Wellbutrin is primarily used as a treatment option for depression, it is sometimes recommended to treat some cases of ADHD.

In this article, Ill explain what Wellbutrin is, how it works, and what its used to treat.

Ill talk about how Adderall works and what its used to treat.

Ill then explain the differences between these two medications, especially as it relates to ADHD.

Ill outline some precautions for both medications, and tell you when you should see a doctor or other healthcare professional.

Started Wellbutrin 150 mg once a day to combat the decreased libido and anorgasmia of my daily Setraline 50 mg for anxiety, which it definitely helped with, but only after increasing to 300 mg daily after 3 weeks on 150 mg. I also started this med to help with my fatigue, lack of energy and lack of motivation, plus my ADD . At the 300 mg dose the fog was lifted and I felt so much better and finally motivated and no longer burdened by every day tasks. Definitely felt jittery at times and had to stop any stimulants like adderall or caffeine to keep my anxiety at bay, but it seemed to make a world of a difference for my motivation and ability to focus. Sadly, after 2 months, the mood/energy boost effect has disappeared. Not sure if I need to consider a dose increase, but worried about increased anxiety symptoms coming with it.

Researchers found out that patients given bupropion were 50% more likely to experience some level of symptom improvement than the control group.

They defined symptom improvement as either improvement noted by the clinician or a 30% improvement in ADHD symptoms.

In controlled trials such as those analyzed here, participants are compared against individuals given a control. Individuals in the control group, given a placebo such as a sugar pill, often can experience improvement even though they aren’t given an active medication. Researchers use this as the baseline for evaluating the effectiveness of a particular treatment.

Main results: We included six studies with a total of 438 participants. Five studies were conducted in the USA, and one in Iran. All studies evaluated a long-acting version of bupropion, with the dosage ranging from 150 mg up to 450 mg daily. Study intervention length varied from six to …

Individuals given bupropion were 78% more likely to experience improvement noted by their doctor or clinician, as represented by the CGI scale. This is a more restrictive definition of improvement that may be more specific than the previous definition.

While fewer patients reached this level of improvement, the ones that did were much more likely to have been taking bupropion than a placebo control.

After going through 2 sets of meds that did not work and made me extremely depressed, I took Wellbutrin at 150mg. Within 30 mins I was bouncing off the walls with energy and positivity. I was like this for a few days and then my body adjusted. I still felt really good and I could tell my brain was leveling back to normal. Ive been on it for almost 5 weeks now. I think it does its job to keep me from falling through the cracks. I still struggle with anxiety but thats a separate issue I am addressing in Therapy. I am just glad I finally got something that gave ME no side effects. The others wrecked me. I experienced a shift in a week manic ep and now I think im in a depressive ep, but its not harsh. I am just unmotivated and bored. Maybe I will increase the dose and see how that works.

Heart Problems Associated With Adderall And Wellbutrin

Pin on adhd

Wellbutrin has been linked with high blood pressure, sometimes to emergency levels. While this effect is most prevalent in people who already have a history of high blood pressure, it also appears more in patients using the drug to quit smoking. The FDA states, There is no clinical experience establishing the safety of Wellbutrin in patients with a recent history of myocardial infarction or unstable heart disease. Therefore, care should be exercised if it is used in these groups.

Adderall and other stimulants have long been the source of debate due to their cardiovascular health implications. Common heart-related side effects of Adderall include hypertension and elevated heart rate. One study found that 3% of otherwise healthy adults experienced adverse cardiovascular effects, including severe high blood pressure, heart palpitation, or tachycardia when using Adderall. When combined with the dangers that Wellbutrin can pose to a patients heart health, the potentially negligible cardiovascular symptoms presented by Adderall can quickly become far more life-threatening. Taking Wellbutrin and Adderall together adds significant risk of long-term heart problems and sudden death by a heart attack.
